Equicare installs EQUICARE CS™ at PeaceHealth St. Joseph Medical Center

Vancouver, BC – July 11, 2013 – Equicare Health, the industry leading provider of care coordination software, announces the installation of EQUICARE CS at PeaceHealth St. Joseph Medical Center in Bellingham, WA. PeaceHealth, a not-for-profit health care system with medical centers throughout the Northwest, installed the software in its Bellingham Center in May 2013 to support the launch of a new Cancer Survivorship program. Through the survivorship program, PeaceHealth will now provide patients with treatment summaries, long term follow-up plans, and educational resources to improve patient engagement and long term clinical outcomes following cancer treatment.

EQUICARE CS™ is a web-based software solution that enables clinics to rapidly provide patients with structured, long-term survivorship care plans modeled on industry guidelines. Care plans may include treatment summaries with test results, follow-up surveillance plans, and diagnosis specific educational materials such as advice on managing side and late effects, exercise and nutrition. Equicare’s tight integration with existing oncology information systems allows for data to be automatically populated into the survivorship program without manual entry. Patients engaged in proactive cancer survivorship programs have been shown to have an improved ability to detect and address recurrences and treatment related late effects earlier.

“We’re extremely pleased to augment the current services offered at PeaceHealth St. Joseph Medical Center Cancer Center with the launch of the Cancer Survivorship program,” states survivorship program Physician Assistant, Kelli Cole-Vadjic, MS, PA-C. “The survivorship tools in EQUICARE CS enable us to provide polished, comprehensive treatment summaries for our patients and create follow-up plans based on current guidelines without major administrative overhead. Although the program is still new, we are already receiving positive feedback and look forward to offering survivorship care plans to more patients in future.”
