EQUICARE CS meets CoC Continuum of Care Services Standards 3.1, 3.2 and 3.3 – patient navigation, psychosocial distress screening and survivorship care plan. ECS includes a targeted action lists for monitoring CoC compliance which identifies compliance issues relative to CoC standards and provides analytics to report results to your hospital cancer committee and ACoS.

Standardized follow-up template to track lung screening patients and collect data that can be uploaded to the ACR Lung Cancer Screening Registry.

Costs and  Limitations:

  • EQUICARE CS™ (ECS) involves a one-time initial software license fee based on the contractual number of active patients allowed in the system. Additional costs include a one-time implementation fee which includes on-site installation, project management and remote training. A customer may incur other costs if they require HL7 interfaces, hardware, third party software or system hosting. Most of these costs would be one time, although hosting will involve an annual cost.

  • DataMotion Direct is the Health Information Service Provider (HISP) directly supported by EQUICARE CS™ . Customers will require a Direct address, included in the one-time license fee, in order to allow patients to transmit a summary of their health information to a third party electronically . Customers do not enter into a contract with DataMotion Direct and will not incur additional costs unless they require more than one Direct address. A customer may want more than one Direct address if they have more than one facility and want health information to be identified as being sent to another care provider from a specific facility. The customer would obtain additional Direct addresses from Equicare Health Inc. The use of a Direct address does not limit the exchange of information with other care providers as DataMotion Direct allows communication with care providers who may use other HISP’s.

  • Customers can purchase ongoing software support fees covering software maintenance and upgrades, with the following technical, contractual and operational limitations: does not include maintenance of hardware or third party software.

  • ECS can also be licensed on a Software As A Service basis, in which case the customer is invoiced quarterly for the number of patients activated in the ECS system. The same services and limitations remain in effect with the SAAS model.
