Self Regional Healthcare implements EQUICARE CS™ for their survivorship program
01 August, 2010
Self Regional Healthcare purchases Cogent Health Solutions’ EQUICARE CS™
Self Region Healthcare has chosen Cogent Health Solutions’ EQUICARE CS for the management of their cancer survivors aimed at improving overall cancer survivors’ care and clinic efficiency.
By using EQUICARE CS, Self Region Healthcare is able to provide cancer survivors with a comprehensive Care Plan, as well as a follow-up plan at the completion of treatment. Educational material included in the Care Plan provides patients with information on managing side effects and health promotion activities, such as nutrition, exercise, and smoking cessation. The structured follow-up plan ensures that survivors know when to expect follow-up activities, which helps ease stress on the patient. Through the EQUICARE CS dashboard, Self Regional staff can monitor the upcoming scheduled activities for survivors and generate reminder calls to confirm appointments, improving clinic efficiency through a reduction in missed appointments.
About Self Regional Healthcare
Self Regional Healthcare got its start in 1951 as Self Memorial Hospital. The vision of local business leader and philanthropist James C. Self was to provide one of the most advanced hospitals in the nation, and he personally oversaw the construction and recruited top physicians from across the country.
Mr. Self’s commitment to advanced care continues today, as the hospital’s mission is “to bring advanced care closer to home.” The difference is Self Regional has evolved into a major referral and medical center providing advanced health services to more than a quarter of a million people in a seven-county area.
Self Regional’s approach is to bring together highly trained physicians and team members, sophisticated technology and compassionate care. Personalized treatment is offered in the Self Regional Healthcare Cancer Center, which provides advances services for the seven-county region. The hospital is committed to maintaining this goal, as evidenced by a new $25 million Cancer Center now under construction and scheduled for completion in the spring of 2011. Whether it’s clinical oncology or radiation oncology services, Self Regional offers patients the opportunity to remain close to home while battling the disease.
Each month, physicians, hospital management and hospital team members assemble to evaluate the performance of the Cancer Center, which is certified by the American College of Surgeons. In addition to the quality services provided at the center, Self Regional offers and coordinates community wellness and screening programs, home health and community-based hospital programs.
Access is also provided to support groups that bring together people affected by cancer. Through these groups, patients can share their experiences and find comfort in shared understanding.
The Tumor Board and Cancer Registry keeps Self Regional informed about the various aspects of cancer treatments and abreast of the latest recommendations for new and improved care. For more about Self Regional Healthcare and its Cancer Center, go to www.selfregional.org.
