Queen’s Cancer Center Survivorship Program reaches 1 year milestone

Queen’s Cancer Center has been using EQUICARE CS™ for the past year in to manage their cancer survivors. By using EQUICARE CS they are able to provide their cancer survivors with a comprehensive Care Plan as well as a follow-up plan at the end of their treatment. Education material included in the Care Plan provides information on managing side effects and health promotion activities, such as nutrition, exercise, and smoking cessation. The structured follow-up plan ensures that survivors know in advance of their scheduled appointments. Utilizing EQUICARE CS dashboard, the Survivorship Navigator is able to monitor the upcoming scheduled activities for the survivors and generates reminder calls to ensure appointments are kept.
