Equicare Health installs EQUICARE CS™ at the Patricia D. and M. Scot Kaufman Cancer Center

Vancouver, BC, April 3, 2014, Equicare Health, the leading provider of care coordination software, announces the installation of EQUICARE CS at the Kaufman Cancer Center at University of Maryland Upper Chesapeake Medical Center based in Harford County, Maryland. The software was installed in December 2013 in order to improve the patient experience both during and after treatment by providing long term cancer survivorship care plans, patient navigation services, and an online patient portal.

EQUICARE CS (ECS) is a web-based software solution used by cancer centers around the world to coordinate care for cancer patients.  ECS includes tools to provide patients with survivorship care plans that include treatment summaries, lab and test results, follow-up plans, care team contact information and diagnosis specific educational materials such as advice on managing side and late effects, exercise, and nutrition. Patients engaged in proactive cancer survivorship programs may be able to detect and address recurrences and treatment related late effects earlier, for improved clinical outcomes.

Equicare’s navigation & distress management tools enable organizations to track and manage barriers to care, to ensure patients and their families have the resources they need.  By routinely monitoring patient distress through assessments and questionnaires, organizations are better able to identify areas where the patient experience can be improved.

The online patient portal provides patients with secure access to a consolidated appointment calendar, lab and test results, educational materials, and contact information for their care team.  Through the portal patients have the ability to request and reschedule appointments, set up email or text message appointment reminders, complete health assessments, and securely message members of their care team.  Equicare’s patient portal has been reviewed by the Drummond Group and is certified for Meaningful Use.
