Equicare Health completes installation of cancer survivorship and navigation software at 13 e+CancerCare sites

Vancouver, BC – Equicare Health Inc, a leading provider of care coordination software has completed the installation of EQUICARE CS™ at 13 e+CancerCare facilities.  e+, one of the leading operators of outpatient cancer care centers offering a variety of cancer imaging, chemotherapy, and radiation therapy services, kicked off the project with Equicare on July 18, 2014, and all centers were running live software by October 18, 2014.  e+ currently manages over 1,000 patients through Equicare’s systems.

Equicare employed a “train the trainer” approach to rapidly provide face-to-face training for small groups in all five of the e+ regional markets.  The software was also configured to interface with each care center’s ARIA oncology information system to eliminate duplicate data entry.  This streamlining enabled the rapid creation of survivorship care plans, which provide a comprehensive overview of a patient’s cancer journey and a schedule of recommended activities following treatment through to end of life.  Care plans are given to patients upon completion of treatment, and include all elements recommended by the Institute of Medicine: contact information for the care team, a treatment summary, relevant educational materials, and a long term follow-up plan.

Patients under e+’s care are invited to register for the new patient portal at their first appointment and encouraged to use the portal to complete online questionnaires and interact with the information in the care plan.  The system provides patients with reminders for upcoming appointments and recommended follow-up activities via email and/or text messages, to encourage a healthy lifestyle with appropriate medical check-ups.
