Equicare’s licensing agreement with NCCN enables us to use their Clinical Practice Guidelines in Oncology as a template for the survivorship care plans included in EQUICARE CS.
The FACIT Measurement System and FACIT.org (www.facit.org) manages the distribution and information regarding administration, scoring and interpretation of a range of questionnaires that measure health-related quality of life for people with chronic illnesses. The FACIT mission is to promote use and application of these measurements as a means of improving healthcare for chronic illness. Through Equicare’s licensing agreement with FACIT, we are able to include their questionnaires and scoring in English and Spanish within EQUICARE CS.
